Is Laparoscopic Nephrectomy The Preferred Approach In Xanthogranulomatous Pyelonephritis?
In this report of 10 standard transperitoneal laparoscopic versus 15 open nonrandomized retrospective nephrectomies, the major/minor complication rates were 10%/20% in the laparoscopic group and 20%/27% in the open group and the hospital stay was reduced from 8.2 days among the open patients to 3.8 days among the laparoscopic patients.
The surgery took 1.3 hours longer to do laparoscopically (i.e. 3.8 hrs). Of note, the conversion rate in the laparoscopic group was 20%. These are far better results than were obtained in the initial series on laparoscopic nephrectomy for XGP in which none of the typical benefits of laparoscopy were evident (i.e. no difference in hospital stay, analgesic use, or convalescence) and the complication rate was 60%; the conclusion at that time was that this condition was "off-limits" for laparoscopy. With the increasing skill of laparoscopic surgeons worldwide, even nephrectomy for XGP is now feasible and reasonable.
