UroToday.com - Dr. Valentini, et al. from Paris, France reviewed the role and tolerability of urodynamics in the elderly. In an analysis of 67 women with a mean age of 83 years, they found a higher incidence of detrusor overactivity as compared to women less than 80 years old (42% vs. 32%). Other statistically significant findings included a higher incidence of rhythmic rectal contractions and lower incidence of intrinsic sphincter deficiency.Dr. O'Connor and colleagues from the Medical College of Wisconsin evaluated the efficacy and safety of using intravesical botulinum toxin A (BTX-A) injections in elderly patients. They reviewed a series of ten patients with a mean age of 75 years undergoing BTX-A with varying dosage, concentrations, and trigonal injections. Subjective improvement of >50% was noted in 50% of patients with an overall decrease in pad usage of 50%. There were no significant perioperative complications, although 90% of their patients experienced postoperative elevations of residual volumes >150mL, all of which were managed conservatively with resolution in 3 months.
