Universal Health Care Impossible Without Reining In Costs, Op-Ed Says

"Universal coverage has dominated both the news media's coverage of the Democratic presidential candidates' reform ideas and the candidates' own jockeying over those ideas," columnist David Leonhardt writes in a New York Times opinion piece. "But if we are really at the start of a once-in-a-generation push to fix health care, we need to be clear about the true problem," he continues, adding, "The main reason so many people lack health insurance is because of its cost. And a big reason for that cost is the explosion of expensive, medically questionable care."

He writes that the leading Democratic candidates all have proposed "the creation of a national institute to figure out which kinds of medical care actually work," the findings of which could then be used by Medicare, private insurers and other payers to "determine when a procedure or a drug would be covered." However, Leonhardt cautions that "we shouldn't be naïve: a lot of people would lose if medical care came to be based more on what actually worked." He also notes that drug companies, medical device makers and doctors profit from costly new treatments, while "many patients demand the latest, most expensive procedure, regardless of the evidence."

Leonhardt continues, "So reforming the system will require a fight -- not just over the meaning of the word 'universal' but also over finding tough, sensible ways to save money." He concludes, "The simple truth is that medical spending can't continue to rise at its current rate. Somehow, we need to make choices" (Leonhardt, New York Times, 6/6).
