Does Tumor Size Or Microvascular Invasion Affect Prognosis In Patients With Renal Cell Carcinoma?

UroToday.com- Tumor size has been shown to directly correlate with patient outcome in patients with renal cell carcinoma (RCC) although the absolute size where tumors demonstrate increased aggressiveness remains a subject of great debate. The presence of lymphovascular invasion in otherwise localized RCC tumors has been associated with an adverse outcome, although again, this remains a subject of controversy with conflicting data in the literature. Here, Miyagawa and colleagues examine these parameters as predictors of outcome in their RCC patient population.

The authors report on 162 patients who underwent radical nephrectomy for localized or locally advanced RCC over a 13 year period. Median follow-up for the group was 97 months. In univariate analysis and multivariate, the authors found that a 5 cm tumor size cutoff significantly differentiated patients with regards to recurrence free (p=0.004) and overall survival (p=0.001). The presence of lymphovascular invasion however, was not a significant predictor of outcome in either univariate or multivariate analysis.

The authors conclude that the 5.0cm cutoff might be a more appropriate size distinction to predict outcome in patients with localized RCC.
