Long-Term Androgen-Deprivation Therapy Results In Metabolic Syndrome In Prostate Cancer Patients

UroToday.com - According to a report in the August 20, 2006 issue of the Journal of Clinical Oncology, prostate cancer (CaP) patients treated with long-term androgen-deprivation therapy (ADT) are at risk for metabolic syndrome and possibly more cardiovascular events. Dr. Braga-Basaria from Johns Hopkins University was first author on the study.

Existing data demonstrates that ADT results in hypogonadism and increased risk of type 2 diabetes mellitus. Furthermore, death from cardiovascular disease (CVD) is the most common cause of non-CaP death and the development of insulin resistance, which occurs in CaP men on ADT, is associated with CVD. This study sought to evaluate the prevalence of metabolic syndrome in men on long-term ADT (ADT group) by comparison to CaP men with rising PSA levels but not on ADT (non-ADT group) and normal age-matched controls.

The definition of metabolic syndrome was if 3 of the following 5 criteria were met: fasting glucose level more than 110mg/dl, serum triglyceride level >150mg/dl, serum high-density lipoprotein level (HDL) 102cm and blood pressure >130/85mmHg. At study entry, the ADT group had significantly higher body mass index and fasting glucose levels compared to the other 2 groups. More than half of the men (55%) met the criteria for metabolic syndrome compared with 22% and 20% of men in the non-ADT and control groups, respectively.

Men in the ADT group had significantly higher overall prevalence of abdominal obesity, hyperglycemia and hypertriglyceridemia compared to the other 2 groups. There was no significant difference in the prevalence of low HDL levels and hypertension between the 3 groups. These data demonstrate that metabolic syndrome is present in 55% of men on long-term ADT therapy, thus predisposing them to CVD.
