Louisiana Should Overhaul 'Outdated' Charity System; Fund Private, Community Hospitals, Draft Report Says

Louisiana should close its "outdated" rural charity hospitals and transfer a share of the tax dollars that fund health care for the uninsured to the state's private and community hospitals, according to recommendations from a draft report by the Public Affairs Research Council, the New Orleans Times-Picayune reports. The draft report says the state's Charity Hospital system, managed by Louisiana State University, is "outdated, uncommon and begs for reform." The report contends that the charity system is too closely associated with medical education. "This inefficient union results in a two-tiered system of care in which the uninsured are left to seek care in an understaffed, underfunded network of 10 charity hospitals scattered throughout the state," the report states. The report recommends the state maintain teaching hospitals in New Orleans, Baton Rouge and Shreveport, where medical students and post-graduates are trained, but the "size of these hospitals should be limited to adequately serve the medical schools' missions of education and research while filling niche patient care needs in the regions they serve." Seven other hospitals in the system that primarily focus on patient care instead of teaching should be transferred to local oversight or sold within two or three years, according to the draft report. The report also recommends that funding for safety net care be allocated in a way that "follows the patients," whether they receive care at private or public hospitals. The final report is expected in about three weeks. LSU Response
LSU officials said the draft report does not detail how care for the uninsured would be funded if the charity system is reduced or eliminated. "The report's recommendations would have us destroy rather than improve a working system upon which around a fifth of the population depends," LSU said in response to the draft report, adding, "PAR advocates the massive closure or divesture of charity hospitals but without identifying a feasible means to fund and organize a viable alternative system to provide care to the uninsured." LSU also said paying insurance premiums for uninsured residents would cost the state three times as much as the current charity system (Moller, New Orleans Times-Picayune, 3/7).
