Street Ketamine-Associated Cystitis And Upper Urinary Tract Involvement: A New Clinical Entity
ORLANDO, FL (UroToday.com) - Ketamine, 2-(2-chlorophenyl)-2-methylamino-cyclohexan-1-one, better known on the streets as "Kit-Kat" or "Special K," is a non-competitive antagonist of the NMDA receptor and has found a home on the illicit drug market due to its ability to produce a state known as dissociative anesthesia (aka: "K-hole").
The authors report a series of 54 ketamine abusers with severe lower urinary tract symptoms that included frequency, small-volume voids, urgency, dysuria, and painful hematuria. 57% had upper tract involvement exhibited by hydronephrosis! Patient evaluation with cystoscopy and bladder biopsy revealed inflammation.
Urodynamic findings were quite impressive -- average cystometric capacity was only 138 cc. Decreased bladder compliance and detrussor instability were documented. 11% of patients had renal insufficiency and required at least temporary upper tract decompression with percutaneous nephrostomy. One patient underwent augmentation enterocystoplasty.
Clearly, when it comes to the GU tract, Special K is not OK.
